sadly, many. but there are also many who change their minds due to people telling them ';they're beautiful no matter what'; personally i think either way is your choice.

All cosmetic surgery is for self esteem. By definition it adds nothing functional.
Parents who allow purely cosmetic surgery send the wrong message. If you have a deviated septum, get a nose job. If you want a nose like Brittany's, forget it.
In what part of the world? Collectively, I would say not that many, as it would require parental consent and not many parents would allow their child to go through with surgery.
